I've been using Absolute Media inc for years for getting my company's CD and DVD duplication done. Their reps are helpful and the silk screen print is outstanding. They're happy to accommodate small and large runs, and it's always a fast turn time.
I haven't had an opportunity to use them for offset or USB yet, but maybe that will come up on a future job.
