My 15 year old with an Autism Spectrum Disorder was given a doctors note for school excusing her from school. She carried the note in her backpack for 2 weeks and got it out in class and traced it with a dark green sharpie so she would not forget to turn it in. She forgot anyway. 2 weeks later at an IEP meeting with 7 other school personnel present, I retrieved the note from my daughter's backpack and carried it to the office. The secretary wanted to know why it was traced, I replied I do not know but my daughter has had it for 2 weeks and forgot to turn it in. 9 days later I receive a certified letter from Dr. DeLeon dismissing my daughter as a patient stating I (the parent) had altered a document. I was shocked at this behavior. He also states in his letter that I cannot see any of the doctors at this facility. This is unprofessional conduct in that there was no HIPAA consent signed allowing the school or doctor to communicate ANY information regarding the patient. Dr. should have called mom to ask if I had any knowledge about this. A 15 year old with Autism spectrum disorder did not realize what she was doing.
