Be very careful if you are asked to provide a blood and/or urine specimen to ""evaluate factors that are not normally monitored by your private physician."" This happened to my wife. They were not able to get her blood specimen (thank goodness), but did take a urine specimen. Several weeks later we received an explanation of benefits from our insurance company that showed submitted charges of $4750 and the lab used was not a PPO. Insurance covered $1852.50 and we owed $2897.50. When we asked the insurance company what the lab work was for, they said it was a drug screen. After negotiation with the lab company (Midas Laboratories LLC0, we only paid an additional $427.50, which represented what the PPO copay would have been, What a rip-off.
