Want to say thank you to Renae, Chris R, Rob R, Mr. G. & Jason. I was so happy and so was my dad. Now he just wants to go home and get moving again.   • Welcome to Kindred Transitional Care and Rehabilitation - Ridgmar. On behalf of our entire team, thank you for considering our nursing and rehabilitation center for you or your family member¿s care.

    From intensive short-stay rehab to longer-term restorative care, our nursing and rehabilitation centers provide a full range of medical and social services to treat and support each of our patients and residents. Many of our residents return home, but for those who cannot, we provide safe, compassionate care in an environment that fosters independence and dignity.


    A Bridge between Hospital and Home


    When patients leave the hospital, they often need continued care to recover completely. That¿s where we come in. We work with you to set a care plan to meet your goals and abilities. Patients typically receive up to three hours of intensive rehabilitation each day, provided by our specially trained interdisciplinary team consisting of:


    * * Nurses
    * Consulting Physician Specialists
    * Registered Dietitians
    * Rehabilitation Therapists
    * Respiratory and Speech Therapists
    * Social Workers


    Our team treats a variety of conditions including:


    * Cardiac or Respiratory Ailments
    * Hip Fracture/Replacement
    * Knee or Other Joint Replacement
    * Neurological Illnesses (Parkinson¿s Disease, Etc.)
    * Post Acute Copd Episode
    * Post Open Heart (Grafts, Valves, Stints)
    * Post Ventilator Care
    * Post-Stroke Recovery
    * Tracheotomy Weaning
    * We Are Focused On Recovery and Results and Continually Strive To Improve Patient Outcomes
