Catholic Community Hospice is dedicated to preserving the quality of life of the dying by restoring their dignity, choices and control. Dedicated care is provided for patients with all terminal illnesses, including, but not limited to, cardiovascular disease, cancer, Alzheimer's, kidney disease, neurological disorders and AIDS.
**How Does the Catholic Community Hospice Team Work?**
Hospice empowers the patient and family to direct their plan of care. A registered nurse is on call and available 24 hours a day. Hospice care can be administered at home or at a home-like setting, such as a long-term care facility.
**Hospice Consists of An Interdisciplinary Team Including:**
*Medical Director
*Registered Nurses
*Social Worker
*Chaplain
*Volunteers
*Therapists
*Healthcare Aides. **Your Hospice Experience May Include Some or All of the Following Services:**
*Consultation,Assessment and Development of Plan of Care with Participation of the Patient,Family and Doctor

*Medical Equipment Arranged Through Third-Party Vendor
*Medication and Pain Management
*Bathing and Other Personal Care
